Humans can't have the visions that was the statement of season 3, it looks like the whole Jasmine's plan started on That vision thing, in some way Jasmine took advantage of this rule, and forced or manipulated The Powers to believe that she wanted to help Cordelia turned her in a half demon being, which that was false, so it looks like Cordelia died at the end, because The powers just restored everything to before the Jasmine manipulation and that was on the episode Birthday when Angel and the gang discovered that she was taking a lot of pain killers, as a way to reduce the damage of the visions, so in some way Cordelia died on Birthday but Jasmine gave her a new deal, and this all over at the end of season four, Cordelia just returned to herself, no more half demon and thats why she ended on a coma, her fate on Birthday.

Jasmine actually came in when Cordelia went to the higher planes. That's when Cordy became possessed. Jasmine got in while Cordy was in the higher planes. The PTB put a protection in that gave her amnesia to keep Jasmine locked. Lorne unlocked Jasmine when he restored her memory at the end of Spin the Bottle.

The powers that be never had anything to do with Cordy and the visions.

Skip was working for Jasmine as far back as season 3. He tells Angel he took a dive during their first encounter (that vision thing)

So yeah when Cordiela dies in Birthday he tricks her into becoming part Demon so that she will ascend to a higher plain (if that's where she went at all). Jasmine was able to enter her body because she was part demon (or maybe she entered her body when she became part demon)

Meh. I don't beleive skip's grandstanding. The powers let Cordy have the visions, although they were rather hands off as far as guidence or consequences of that happening, but if Jasmine had planned as far ahead and was as omnipotent as Skip described, she wouldn't have had such a poorly executed endgame. Think how bungling the beast and evil cordy/beastmaster was. Skip was just trying to get everyone upset while he was captured.
